Therefore, athletes are encouraged to stay hydrated during training and, when given the opportunity, to consume fluids during exercise to limit water and salt consumption. Dehydration is not a medical condition per se (Noakes, 2012), but refers to a decrease in total body water content.

The only real symptom of dehydration is thirst, which acts to induce drinking (Goulet, 2011). In modern endurance events with sufficient fluids, healthy athletes are at limited risk of dehydration (Noakes, 2012). Only when total body water is reduced by about 15%—which occurs in those who lose water for more than 48 hours in the desert—voluntary locomotor activity is completely suppressed, resulting in paralysis (Noakes, 2012).

However, the signs and symptoms of overhydration are obvious. The increase in water leads to lower sodium levels in the blood (hyponatremia), and athletes experience significant changes in brain function, ranging from mild withdrawal to confusion, convulsions, and coma.

Why We Feel Thirsty During Exercise

Diagnosis is confirmed by measurement of sodium concentration in the blood; the lower the value, the greater the fluid overload (Irving et al., 1991; Noakes et al., 2005). Most (if not all) training providers will offer their members the opportunity to rehydrate (before, during and after the session), but this will come in the form of water.

Noakes (2012) noted that confusion arises when the misconception that all athletes who collapse after exercise suffer from dehydration-induced fever is promoted as part of "false hydration science". This confusion dictates that the collapsed person needs to drink more fluids during exercise and be immediately resuscitated with massive IV fluids.

Humans can raise their body temperature slightly (adaptive hypothermia) when forced to exercise without any or optimal fluid replacement.
